Appointments
To schedule an appointment, please call the appropriate clinic for your needs, and explain the reason for your visit. This allows us to schedule the appropriate amount of time for your appointment to fully address your concerns.
Your First Appointment
Please arrive at the appropriate clinic at least 15 minutes prior to your appointment, so that we may collect the information required for establishing your care.
If you are a new patient for the practice, please be prepared to bring:
- Your complete demographic information, include address and phone number
- Insurance card(s)
- Emergency contact information
What To Bring & Expect
Please arrive at the appropriate clinic at least 15 minutes prior to your appointment, so that we may collect the information required for establishing your care.
For each appointment, please be sure:
- To bring your insurance card
- To bring a list of your current medications
- To inform the front desk staff of any change in name, address, telephone number, employment or insurance
- To inform your physician of any changes in medical history
As a courtesy to other patients in our office, please abide by the following rules:
- No smoking
- No heavy perfumes, colognes, or other products that emit a strong odor
- Please turn off your cell phone
Late Arrival, Rescheduling & Cancellations
If you are unable to keep your appointment for any reason, we require at least 24 hours advance notice, or you may be charged for the appointment. If you are assessed this fee, it is required to be paid before your next visit.
If you do arrive late for your appointment, please call to notify our office, as we will need to reschedule.
Emergency Care
If you have an urgent need to be seen at one of our clinics, please call the appropriate clinic as soon as you can, and they will do their best to accommodate you. We do our best to stay on schedule, but there are circumstances that may cause unexpected delays. We do our best to keep these delays to a minimum.
